How we support?

Home care assessment

  • Determine needs
  • Assess home accessibility
  • Recommend and prescribe mobility and transfer equipment
  • Provide assistive bathroom equipment

Fall prevention

  • Assessment of clients mobility including balance, gait, transfers and stair climbing

Professional liaison

  • Liaise with family physician, specialists and family members
  • Accompaniment to doctors appointments
  • Referrals for audiology, physiotherapy and personal support workers

Nursing home/retirement home transition assistance

  • Education on suitable living facilities, arrangement of tours and assistance in planning and transition
  • Assess ongoing need for attendant care services

Occupational Therapy assessment

  • Devise programs and support to reduce social isolation and improve quality of life, home accessibility and function level

We encourage elderly to stay at home which is made possible with added support of assistive devices and additional personnel
